ปัจจัยทางการเมืองที่มีผลต่อรายจ่ายสาธารณะ ด้านการเสริมสร้างสุขภาพและป้องกันโรคของประเทศไทย พ.ศ. 2545-2566

บทคัดย่อ

การจัดสรรรายจ่ายสาธารณะด้านการเสริมสร้างสุขภาพและป้องกันโรคถือเป็นกลไกสำคัญที่สะท้อนถึงความมุ่งมั่นของรัฐในการพัฒนาระบบสุขภาพอย่างยั่งยืน โดยเฉพาะในยุคที่ประเทศไทยเข้าสู่สังคมผู้สูงอายุและเผชิญกับการเปลี่ยนแปลงรูปแบบโรคจากโรคติดต่อเฉียบพลันเป็นโรคไม่ติดต่อเรื้อรัง อย่างไรก็ตาม การตัดสินใจจัดสรรงบประมาณของภาครัฐไม่ได้อิงอยู่บนหลักการด้านสุขภาพหรือความต้องการของประชาชนแต่เพียงอย่างเดียว หากแต่ขึ้นอยู่กับปัจจัยกำหนดรายจ่ายหลากหลายมิติ โดยเฉพาะปัจจัยทางการเมืองที่มีบทบาทสำคัญในการกำหนดทิศทางนโยบายสาธารณะ การศึกษานี้มีวัตถุประสงค์เพื่อ 1) ศึกษาปัจจัยทางการเมืองที่มีอิทธิพลต่อการกำหนดรายจ่ายสาธารณะด้านการเสริมสร้างสุขภาพและป้องกันโรค และ 2) นำเสนอข้อเสนอแนะเชิงนโยบายในการพัฒนาการจัดสรรรายจ่ายสาธารณะด้านการเสริมสร้างสุขภาพและป้องกันโรคให้มีประสิทธิภาพมากยิ่งขึ้น ผู้วิจัยใช้แนวทางการวิจัยเชิงปริมาณด้วยข้อมูลอนุกรมเวลาระหว่าง พ.ศ. 2545-2566 และเทคนิคการวิเคราะห์การถดถอยพหุคูณ โดยบูรณาการทฤษฎีการเงินสาธารณะ ทฤษฎีเศรษฐกิจการเมือง และทฤษฎีการกำหนดรายจ่ายสาธารณะ ช่วงเวลาที่ศึกษาครอบคลุมการเปลี่ยนแปลงทางการเมืองสำคัญหลายครั้งของประเทศไทย รวมถึงรัฐประหาร การเปลี่ยนแปลงรัฐบาล และการปฏิรูประบบสุขภาพ ผลการศึกษาปัจจัยทางการเมืองที่มีอิทธิพลต่อการกำหนดรายจ่ายสาธารณะด้านการเสริมสร้างสุขภาพและป้องกันโรคพบว่าแบบจำลองสามารถอธิบายความแปรปรวนของตัวแปรตามได้ร้อยละ 91.9 (R² = 0.919) โดยความเป็นประชาธิปไตยของรัฐบาล (β = -1.352, p = 0.001) และการกระจายรายได้ทางภาษี (β = -1.037, p < 0.001) มีอิทธิพลเชิงลบต่อรายจ่ายด้านการป้องกันโรคอย่างมีนัยสำคัญทางสถิติ ขณะที่รายจ่ายปีก่อนหน้า (β = 0.040, p = 0.005) มีอิทธิพลเชิงบวก สะท้อนลักษณะการจัดสรรงบประมาณแบบฐานเดิม (Incremental Budgeting) และมีข้อเสนอแนะเชิงนโยบายสำคัญ 5 ประการ ได้แก่ การสร้างกลไกป้องกันการแทรกแซงทางการเมือง การพัฒนาระบบการสื่อสารเชิงประจักษ์ การปรับปรุงระบบการกระจายอำนาจด้านสาธารณสุข การพัฒนาระบบการประเมินผล และการปรับปรุงกลไกการจัดสรรงบประมาณ ผลการศึกษานี้ขัดแย้งกับทฤษฎีประชาธิปไตยแบบดั้งเดิมและสนับสนุนทฤษฎีทางเลือกสาธารณะที่อธิบายว่านักการเมืองในระบอบประชาธิปไตยมักมุ่งเน้นนโยบายที่ให้ผลตอบแทนทางการเมืองในระยะสั้น การศึกษานี้มีนัยสำคัญต่อการพัฒนากลไกการป้องกันการแทรกแซงทางการเมือง การปรับปรุงระบบการกระจายอำนาจด้านสาธารณสุข การพัฒนาระบบการสื่อสารเชิงประจักษ์ และการสร้างความต่อเนื่องของการลงทุนด้านการป้องกันโรคเพื่อสุขภาพประชาชนไทยในระยะยาว
