การฟื้นพลังทางจิตใจของวัยรุ่นที่มีภาวะซึมเศร้า : การวิจัยเชิงคุณภาพ

บทคัดย่อ
ที่มาและวัตถุประสงค์: ภาวะซึมเศร้าเป็นปัญหาด้านสุขภาพจิตที่สำคัญระดับโลก โดยเฉพาะสำหรับวัยรุ่นและวัยผู้ใหญ่ตอนต้น ข้อมูลจาก Mental Health Check-in ของกรมสุขภาพจิต พบว่าเยาวชนไทยอายุต่ำกว่า 20 ปี มีภาวะเสี่ยงซึมเศร้าร้อยละ 29.51 มีการศึกษาที่พบว่าความสามารถในการฟื้นพลังทางจิตใจเป็นหนึ่งในปัจจัยป้องกันและปัจจัยทำนายการฟื้นตัวจากภาวะซึมเศร้าที่สำคัญ แต่การศึกษาเชิงคุณภาพเกี่ยวกับการฟื้นพลังทางจิตใจของวัยรุ่นที่มีภาวะซึมเศร้ายังมีน้อยมากโดยเฉพาะในบริบทสังคมไทย งานวิจัยนี้จึงมีวัตถุประสงค์เพื่อค้นหาความสามารถในการฟื้นพลังทางจิตใจของวัยรุ่นที่เคยผ่านประสบการณ์ซึมเศร้า
ระเบียบวิธีการศึกษา: เป็นการวิจัยเชิงคุณภาพตามแนวปรากฏการณ์วิทยา กลุ่มเป้าหมายเป็นวัยรุ่นตอนปลาย อายุ 18 – 25 ปี ที่เคยมีอาการของภาวะซึมเศร้าต่อเนื่อง 3 เดือนขึ้นไป และเคยได้รับการรักษา ปัจจุบันไม่มีอาการมานานกว่า 1 ปี การเลือกผู้ให้ข้อมูลเป็นการเลือกแบบเฉพาะเจาะจง (Purposive Sampling) ตามคุณลักษณะของที่กำหนด การเข้าถึงผู้ให้ข้อมูลหลักเริ่มจากผู้ให้ข้อมูลคนแรกที่ได้รับการแนะนำมา และใช้วิธีการ Snow Ball โดยผู้ให้ข้อมูลคนแรกแนะนำผู้ให้ข้อมูลคนต่อไป ผู้ให้ข้อมูลค่อยๆ ปรากฏตัวขึ้นรวม 7 คน เครื่องมือการวิจัย คือแนวคำถามกึ่งโครงสร้างสำหรับการสัมภาษณ์เชิงลึกที่ผู้วิจัยสร้างขึ้น วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์เนื้อหา มีการตรวจสอบยืนยันผลการวิจัยโดยผู้ทรงคุณวุฒิ
ผลการศึกษา: สรุปประสบการณ์การฟื้นพลังทางจิตใจของวัยรุ่นจากภาวะซึมเศร้า 5 ประเด็น เริ่มต้นจาก 1. การยอมรับตนเองและเปิดใจรักษา 2. การมีเป้าหมายและความหมายในชีวิต ประกอบด้วย 2.1 การมุ่งมั่นที่จะหายจากความซึมเศร้า 2.2 การมองเห็นเป้าหมายที่มีความหมายในอนาคต 2.3 การมีเป้าหมายให้ชีวิตในทุกวัน 3. การสร้างประสบการณ์ใหม่และเปลี่ยนสิ่งแวดล้อมใหม่ ประกอบด้วย 3.1 การสร้างยาความสุขด้วยกิจกรรมใหม่ๆ 3.2 การพบคุณค่าตัวเองจากการสร้างสุขให้คนอื่น 3.3 การเชื่อมโยงกับโลกภายนอกและออกสู่สิ่งแวดล้อมใหม่ 4. ความสัมพันธ์ที่เกื้อกูลและการสนับสนุนทางสังคม ประกอบด้วย 4.1 การแสวงหาและได้รับการสนับสนุนจากเครือข่ายทางสังคม 4.2 การฟื้นคืนความสัมพันธ์กับครอบครัว 4.3 การสนับสนุนจากครอบครัว 4.4 การสร้างความสัมพันธ์ใหม่ที่เกื้อกูล 5. การปรับวิธีคิดและวิธีการใหม่ในการรับมือกับปัญหา ประกอบด้วย 5.1 เปลี่ยนมุมคิดใหม่ให้ไม่ทุกข์ 5.2 เปลี่ยนการรับมือใหม่ในสถานการณ์ต่างๆ
การประยุกต์ใช้: ครู อาจารย์ บุคลากรการศึกษา นักจิตวิทยา ผู้ให้การปรึกษาวัยรุ่น สามารถนำองค์ความรู้นี้ไปใช้ในการออกแบบแนวทางการดูแลทางด้านจิตใจ การพัฒนาโปรแกรมเสริมพลังทางจิตใจสำหรับวัยรุ่นเพื่อป้องกันปัญหาในอนาคตได้
บทสรุป: วัยรุ่นไทยที่มีภาวะซึมเศร้าสามารถฟื้นพลังทางจิตใจตนเองจากภาวะซึมเศร้าได้ โดยเริ่มจากการยอมรับตนเองเพื่อเข้าสู่การรักษา การมีเป้าหมายชีวิต การพาตัวเองออกสู่สังคมใหม่ การสร้างความสัมพันธ์ที่เกื้อกูล การเข้าถึงการสนับสนุนทางสังคมในหลายมิติ และการปรับมุมมองความคิดใหม่
