A questionnaire was used to collect data, and analyzed for predictive factors using multiple linear regression. 2) Action research study, to develop a model for preventing falls in the elderly through a community participation process. These include planning, implementing, observing, and reflecting. Sample groups include stakeholders, a total of 48 people, analyzed data by collecting, categorizing, and content analysis. 3) Evaluating the model effectiveness, by using paired sample t-test statistics, analyzed from the data collected from a sample group of 90 people.

They were able to jointly explain the variations in factors affecting falls among the elderly for 55.7 percent (R2adj=0.557, p < 0.001) (2) Model for preventing falls in the elderly is “PHOYAI Model” consisting of 1) Participation 2) Health Assessment & Human-Relations 3) Opportunity 4) YOURSELF 5) Accountability & Appreciation 6) Initiative & Improvement (3) The results of the evaluation found that the average knowledge, perception of disease risk, perceived benefits of the practice, intention, participation. perception of one's abilities, social support, and behaviors to prevent falls in the elderly, after the experiment were higher than before the experiment, with statistical significance at the 0.05 level.
