การประเมินผลโครงการเอ็นซีดีแอดโฮมเพื่อการรักษาและดูแลผู้ป่วยโรคเบาหวานและความดันโลหิตสูง ในอำเภอวัฒนานคร จังหวัดสระแก้ว

การประเมินผลโครงการเอ็นซีดีแอดโฮมเพื่อการรักษาและดูแลผู้ป่วยโรคเบาหวานและความดันโลหิตสูง อำเภอวัฒนานคร จังหวัดสระแก้ว ประยุกต์ใช้โมเดลตรรกะ ประกอบด้วย ปัจจัยนำเข้า กระบวนการ ผลผลิต และผลลัพธ์ ดำเนินการในกลุ่มผู้ปฏิบัติงานโครงการที่ดูแลผู้ป่วย ณ สถานบริการและที่บ้าน กลุ่มตัวอย่างคือผู้ปฏิบัติงานหลักในโครงการ 26 คน เก็บข้อมูลด้วยการสัมภาษณ์เจาะลึก สนทนากลุ่ม และรวบรวมข้อมูลจากเอกสารในประเด็นปัจจัยนำเข้าและกระบวนการ และจากระบบบริการในประเด็นผลผลิตและผลลัพธ์ วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา การแจกแจงความถี่ และร้อยละ
ผลการวิจัยจากข้อมูลการรับบริการ ด้านผลลัพธ์พบว่าหลังเข้าโครงการสัดส่วนผู้ป่วยเบาหวานที่มีระดับ HbA1c ในเกณฑ์ควบคุมได้ลดลง สัดส่วนผู้ป่วยความดันโลหิตสูงที่มีระดับความดันโลหิตในเกณฑ์ควบคุมได้ใกล้เคียงกับก่อนเข้าโครงการ และไม่พบผู้ป่วยเข้ารักษาซ้ำในแผนกผู้ป่วยใน ด้านผลผลิตพบว่าจำนวนครั้งเข้ารับบริการของผู้ป่วยลดลงเล็กน้อย ส่วนการขาดนัดเพิ่มขึ้นในช่วงหลัง ผลการวิจัยจากเอกสารร่วมกับสัมภาษณ์ผู้ปฏิบัติงานหลักด้านปัจจัยนำเข้าและกระบวนการพบว่าการอบรมฟื้นฟูอาสาสมัครสาธารณสุขประจำหมู่บ้านไม่ต่อเนื่อง เกณฑ์การวินิจฉัยและคัดเลือกช่วยลดผลกระทบในผู้ป่วยแต่จำกัดจำนวนผู้เข้าร่วมโครงการ การใช้แอปพลิเคชันในการดูแลผู้ป่วยลดลง มีการปรับกระบวนการเยี่ยมบ้านและส่งยาถึงบ้านในช่วงการระบาดของโควิด 2019 งบประมาณไม่เพียงพอ และระบบสารสนเทศบางส่วนซ้ำซ้อนกับระบบเดิม
