การใช้ยานอกข้อบ่งใช้ในประเทศไทย (Off-label drug use in Thailand)

Abstract

ประเทศต่าง ๆ มีระบบควบคุมยาที่จำหน่ายในประเทศอาศัยหลักการประเมินผลได้-ความเสี่ยงจากการใช้ยา  เพื่อป้องกันอันตรายที่จะเกิดจากการใช้ยาของผู้บริโภค เช่นการขึ้นทะเบียนยาก่อนอนุญาตให้จำหน่าย การเฝ้าระวังปัญหาจากการใช้ยาภายหลังจากที่ยาออกสู่ท้องตลาด การใช้ยาตามข้อบ่งใช้ที่ผ่านการพิจารณาของหน่วยงานรัฐจึงมีความเสี่ยงต่ำที่จะเกิดอันตราย  แต่การสั่งใช้ยาเป็นดุลพินิจของผู้ประกอบวิชาชีพ ไม่ได้มีกฎหมายควบคุม  ดังนั้นในทางปฏิบัติจึงพบว่าบางครั้งมีการใช้ยานอกข้อบ่งใช้ ด้วยเหตุผลหลายประการ อาทิเช่น การขยายการใช้ยาไปใช้รักษาในอาการซึ่งใกล้เคียงกับอาการที่ผ่านการรับรอง หรือกลุ่มผู้ป่วยซึ่งไม่มีรายงานการทดสอบยาเพราะหลักจรรยาบรรณของงานวิจัยไม่สามารถทำการทดสอบยาในกลุ่มผู้ป่วยเหล่านี้ ก่อนขึ้นทะเบียนได้

ประเทศไทยก็พบการใช้ยานอกข้อบ่งใช้ เช่น ยาที่ผลิตโดยโรงพยาบาลของรัฐซึ่งได้รับการยกเว้นไม่ต้องขึ้นทะเบียน  หรือ การพิจารณายาเข้าในบัญชียาหลักแห่งชาติ ซึ่งมีวัตถุประสงค์เพื่อจัดทำบัญชีของยาที่ต้องใช้ในการป้องกันและแก้ไขปัญหาสุขภาพของประชาชนชาวไทยโดยครอบคลุมโรค สาเหตุของโรคและการรักษาสุขภาพของคนไทยในระดับจำเป็นขาดไม่ได้ แต่ประหยัดและคุ้มค่า จึงทำให้มีบางรายการที่เป็นยานอกข้อบ่งใช้ เช่น ยา Bevacizumab  ใช้สำหรับ age-related macular degeneration และ ใช้สำหรับ diabetic macular edema และ ยา Nortriptyline hydrochloride ใช้ในการเลิกบุหรี่ ดังนั้น ประเด็นการสั่งใช้ยานอกข้อบ่งใช้จึงต้องพัฒนาระบบที่เกี่ยวข้องทั้งหมดต่อไป เพื่อให้การใช้ยาของผู้บริโภคมีความปลอดภัยและมีความคุ้มค่ามากขึ้น

In many countries, drug system is regulated through risk-benefit analysis to prevent dangers from consumers’ drug use such as pre-marketing control and surveillance program for consumers' safety. Thus on-label drug use has a low risk. On the other hand, drug dispensing depends on the discretion of the practitioners as there is no law to regulate it. In consequence, in practice there is off label drug use for many reasons: extension to milder forms of an approved indication, extension to related conditions, unapproved subpopulations; due to research ethics, drugs cannot be tested on these groups of subpopulation prior to licensing.

            In Thailand, off label drug use was found in various forms. For instance, there are drugs which are produced in all the government hospitals, which do not require licensing or registering into the National List of Essential Medications, the purpose of which is to create a formulary of drugs used for preventing and solving health problems of Thai nationals. This formulary comprises necessary and cost-effective drugs that tackle the disease, causes of diseases and health maintenance, thus rendering some of them off label. The example items of off label drug use were Bevacizumab indication for age-related macular degeneration or diabetic macular edema and Nortriptyline hydrochloride for smoking cessation. As a consequence, off label drug dispensing requires various relevant systems to ensure consumer’s safety and cost effectiveness.
