Abstract

บทความวิชาการนี้ใช้การทบทวนและสังเคราะห์วรรณกรรมเพื่อวิเคราะห์การรับรู้และการจัดการโรคอ้วนในเด็กของผู้ปกครอง โดยใช้แบบจำลองความเชื่อทางสุขภาพ (Health Belief Model: HBM) ควบคู่กับกรอบเศรษฐกิจการเมืองของอาหาร (political economy of food) เพื่ออธิบายความสัมพันธ์ระหว่างการรับรู้ของผู้ปกครองกับปัจจัยเชิงโครงสร้างที่กำหนดพฤติกรรมด้านอาหาร บทความเสนอว่าโรคอ้วนในเด็กไม่ควรถูกอธิบายว่าเป็นเพียงผลของพฤติกรรมการเลี้ยงดูของผู้ปกครอง หากแต่เป็นผลของปฏิสัมพันธ์ระหว่างการรับรู้ของผู้ปกครองกับสภาพแวดล้อมทางอาหารและเงื่อนไขเชิงโครงสร้างของระบบอาหาร การสังเคราะห์วรรณกรรมชี้ให้เห็นว่าแบบจำลองความเชื่อทางสุขภาพมีประโยชน์ในการอธิบายการรับรู้ของผู้ปกครองต่อความเสี่ยง ความรุนแรง ประโยชน์ และอุปสรรคของการจัดการโรคอ้วนในเด็ก แต่หากใช้เพียงลำพังอาจนำไปสู่การอธิบายแบบลดทอนที่เน้นความรับผิดชอบของปัจเจกมากเกินไป ทั้งที่การรับรู้ดังกล่าวถูกหล่อหลอมโดยสภาพแวดล้อมทางอาหาร การขยายตัวของอาหารแปรรูปขั้นสูง การตลาดอาหารต่อเด็ก และข้อจำกัดเชิงโครงสร้างของครัวเรือนเมือง ดังนั้น การจัดการโรคอ้วนในเด็กอย่างมีประสิทธิภาพจึงควรบูรณาการทั้งการส่งเสริมการรับรู้ของผู้ปกครอง การปรับสภาพแวดล้อมทางอาหาร และมาตรการเชิงนโยบาย เพื่อให้การป้องกันและจัดการโรคอ้วนในเด็กมีประสิทธิผลและยั่งยืน


This academic article employs a literature review and conceptual synthesis to examine parental perceptions and the management of childhood obesity by integrating the Health Belief Model (HBM) with the political economy of food framework. The article argues that childhood obesity should not be understood solely as the outcome of parental feeding practices, but rather as the product of the interaction between parental perceptions, the food environment, and the structural conditions of the contemporary food system. The synthesis indicates that the Health Belief Model (HBM) provides a useful framework for explaining how parents perceive susceptibility, severity, benefits, and barriers related to childhood obesity management. However, when applied in isolation, the model tends to overemphasize individual responsibility while overlooking the broader structural conditions that shape parental decision-making. These conditions include the expansion of ultra-processed foods, pervasive food marketing targeting children, and the structural constraints faced by urban households. The article therefore argues that effective and sustainable childhood obesity management requires an integrated approach that combines strengthening parental perceptions, improving food environments, and implementing supportive public policies to achieve effective and sustainable prevention and management of childhood obesity.
